Preferably before the employee leaves the location.

- Treat any first aid
- Complete Supervisor Report
- Have Employee complete Employee Statement
- If Spanish, have translated with the signature of the translator
- Have Witness complete Witness Statement
- If Spanish, have translated with the signature of the translator.
Contact Safety / Manager to assist with care and/or treatment, if needed.
if more than first aid is required
- Notify Safety Manager and Department Manager before the employee leaves the plant for any medical treatment because of an accident
- If an accident/incident happens on the weekend or at night, modify the employee’s job if you can. Keep Safety Manager updated on the status of employee
- Only if it is an emergency or if the employee asks for offsite medical attention should an employee be sent for offsite
- Never, approve medical treatment to any medical facility or doctor on an employee that has gone to the doctor or hospital without them reporting an injury or without proper paperwork. Ask if possible that the employee returns to the plant to talk with the Safety Manager & their Department Manager.
- Accident reports should be filled out by the Supervisor, employee & any witness by the end of the shift
